Rules for mascot naming and colouring competition

For your chance to win a Coles/Myer card in your age section, please colour in the entry form. We have three different age sections, Under 5 years, 5 – 9 years and 10 – 15 years. There will be one prize per age section.

Please write your name clearly on the front and back of the sheet (PDF, DOCX) so that if you win we will be able to find you to give you your prize. Ask Mum or Dad to write down their phone number to help us find you.

For those who are unable to come to the Conference this year, you are able to enter as well. Please print a copy from our website and fill in the name details on the front and also the back and please post to: Albinism Fellowship of Australia, C/- Secretary, PO Box 20500, World Square, NSW 2002.

We would also like you to help us name our Girl and Boy Mascot. If you can think of a good name for either of them, please write it in the section we have put on the entry form. All of the entries will go back to the AFA committee and we will then work out which ones to choose. The most popular names suggested for the girl and boy mascot will be chosen by the AFA Committee. There will be no prize for choosing a name, but if the name you picked is the one we choose, then you know that you helped make our AFA Mascot come to life.

Please post your finished entry no later than Friday 30th September, 2011. If for some reason your entry becomes lost or does not reach us in time, we cannot be held responsible.

The competition winners will be decided during the Melbourne Conference and all prize winners will be notified by mail and their name will be put on the AFA website and in next published Beyond Blonde newsletter. The AFA appointed judges decision will be final and no correspondence will be entered into.
